To what does "heaven and earth" refer?

Genesis 2:4 (ESV)

4 These are the generations of the heavens and the earth when they were created, in the day that the LORD God made the earth and the heavens.

Heaven and earth express the totality of the created order. The whole cosmos is created by God. The whole universe is the outcome of what God has made. So, God is the Lord of heaven and earth and Creator of all in heaven and all on earth and all creatures in both. He is the unique and universal Creator who gives stability and durability to the world. The whole world lies in his hands. He is the only one who can say: the whole world, the whole of the universe is mine.
