What are the three plagues?

Revelation 9:18 (ESV)

18 By these three plagues a third of mankind was killed, by the fire and smoke and sulfur coming out of their mouths.

John specifies that these three are “fire and smoke and sulfur.” Fire is not smoke, and sulfur is not fire; the three are distinct. Yet the three are all of one piece inasmuch as all three are hellishly destructive.
